Any patient, new or existing, may use our confidential Patient Portal for secure communications with our office. The Patient Portal is a web-based system anyone can use to request an appointment, and our existing patients can log in to with a private user name and password to see information that is pulled from your electronic record at the clinic and displayed on the web page. A User Name and Password is required for any function beyond setting an appointment. Print out this authorization form and send to us, or simply call us to obtain more information. Once you are registered, we will email you the access information for logging on our secure portal site.
